School Physicals
Cerro Gordo CUSD #100 School Board has mandated that beginning the 2007-08 school year, school physical and current immunization records for students in Pre K, Kindergarten, grades 5 & 9, and new students from out-of-state are due on the first day of school in August. Students who do not turn completed physical and current immunization records in to the office before the first day of school will not be allowed to attend school until the said physical and current immunization record are on file in the office. Please make sure you make an appointment soon so your child will not be denied attendance in August. School physical forms (and other forms) can be accessed here.
